1. First off you need to create a new form. To Start click on Contact > Add New

3. To add a text box for people to enter choose the correct box, In this case Click on text

Message Body:

Enter the following

[Entry-text]

Under Type "From: [your-name] [your-email] Subject: [your-subject] Message Body: [your-message] Enter the following Entry Text [Entry-text] -- This is a notification that a contact form was submitted on your website ([_site_title] [_site_url])."

16. Click on File attachments and enter [file]
